Our Home Health is a community based home health agency dedicated to delivering compassionate, quality home health care.

As of March 1, 2022: Our Home Health only provides services to individuals that are approved through Medicaid, PASSPORT Waiver, or MyCare Ohio Waiver (UnitedHealthcare & CareSource)

Personal Care Services

Our Home Health provides assistance with personal care. Home Health Aides offer help with daily household maintenance and personal hygiene activities. Personal Care Assistance may include:

Respite Care
Hygiene and Grooming
Meal Preparation
Light Housekeeping
Assistance with Ambulation

Private Duty Services

Update: As of March 1, 2022 all Private Duty services have been put on hold

Our Home Health offers a full range of Private Duty services. We provide nurses who will assess your care needs and nursing assistants who will come to your home to assist you or your loved one with personal care and household chore services. We also provide respite care and companion services.
